About SMRPCC

The Southern Metropolitan Region Palliative Care Consortium is an alliance of all funded palliative care services in the region and a number of associate member agencies with an interest in collaborating regionally on issues around palliative care.

The Victorian End of Life and Palliative Care Framework guides the development of our activities. The 2023-2036 vision for palliative care in Victoria is that:

Victorians can access high-quality palliative and end-of-life care that meets the unique needs of each person and their carers and family. Care is provided through an integrated, responsive and inclusive service system.

The framework has five priority directions are:

• Provide more care at home
• Support people’s holistic needs and those of carers, families and loved ones
• Improve system interfaces and care pathways
• Build a capable and interconnected workforce
• Enhance innovation, digital capability, and use of data

The consortium has an operational plan, which enables the coordinated implementation of activities towards the priority areas.
