SUMMARY:Digital Stories in Residential Aged Care
DESCRIPTION:Providing person-centred care is challenging when staff don’t truly know the person living with dementia — contributing to depression and reduced quality of life. The Know Me intervention addresses this by training healthcare students and interns as volunteers in reminiscence therapy\, partnering them with residents to co-create a 4-minute digital life story. These stories rapidly build staff knowledge of the whole person\, fostering holistic\, person-centred care and meaningfully improving quality of life for residents\, volunteers and staff alike.

SUMMARY:Grief Australia workshop | From Theory to Practice: Applying Grief Models Through Case Studies
DESCRIPTION:This 3-hour interactive workshop bridges the gap between grief theory and real-world practice\, supporting practitioners to move beyond conceptual understanding into confident clinical application. \n\nUsing a range of contemporary bereavement models\, participants will explore how theory can inform assessment\, formulation\, intervention\, and therapeutic stance across diverse grief presentations.\nThe workshop is grounded in case study discussions\, inviting participants to critically reflect on which models fit which clients\, when\, and why.\nAttention is given to complexity\, context\, culture\, and the therapist’s own worldview\, highlighting the importance of flexibility rather than model adherence.\nThrough small-group work\, guided reflection\, and facilitated discussion\, participants will strengthen their capacity to translate grief theory into responsive\, compassionate\, and ethically sound practice.

SUMMARY:Grief Australia | Ethics and Self-Disclosure: Clinical & Practical Implications
DESCRIPTION:Professional boundaries involving self-disclosure may blur when providing care to the dying and bereaved. This “blurring” often causes clinicians and volunteers alike to struggle with whether to share their own experiences with patients\, clients\, and families. Even when it seems important to share these personal stories\, one must decide the exact method for sharing them without the caregiver’s story becoming an emotional burden to the helped individual. Using principles and case examples\, Dr. Hoy will examine how the challenges of self-disclosure can be properly navigated in everyday practice.

SUMMARY:PACOP Fundamentals Workshop | Glen Waverly
DESCRIPTION:Palliative Aged Care Outcomes Program (PACOP) supports aged care homes (ACHs) to provide optimal palliative & end-of-life care for residents and their families. By embedding a framework of routine assessments to identify increasing palliative and end-of-life needs\, PACOP guides care planning to address these needs using an evidence-based practice and evidence-based responses. \nPACOP has developed a model of care that routinely monitors all residents\, thereby assisting in the early identification of palliative and/or end of life care needs. The model has two components which make up the complete PACOP program: \n1. The Profile Collection – routine\, regular\, sustainable assessments that monitor for any baseline changes in the resident \n2. The Outcomes Collection – residents identified with palliative and/or end of life care needs\, are transitioned to this part of the program where closer monitoring of the residents means rapid response to distress\, symptom management and care planning \nThis workshop provides an overview of the foundational knowledge required to implement the PACOP Profile & Outcomes Collection within your ACH.
